How to become an AI whisperer

In Post 4, we saw how AI can be your clinical sidekick—sparking lesson ideas, streamlining documentation, and tackling admin tasks. SLP AI prompt writing is essential for maximizing these benefits. But here’s the truth: AI is only as good as the conversation you start. The magic isn’t in its existence; it’s in how you wield it.

Think of AI as an incredibly intelligent, but frustratingly literal, intern. It operates on your instructions. This is where AI prompt engineering for SLPs comes in—the art and science of communicating effectively with large language models (LLMs). The better your prompt, the more tailored, useful, and relevant the AI’s output will be.

🛑 STOP! The Golden Rule: PHI and AI (Crucial Disclaimer)

Before we dive into prompt perfection, let’s firmly reiterate the absolute necessity: NEVER, EVER input Protected Health Information (PHI) or any client-specific identifying data (such as names, baselines, unique characteristics, or highly specific details that could lead to identification) into general-purpose AI models like CoPilot, ChatGPT, Bard/Gemini’s public interfaces, or similar tools that do not have a specific Business Associate Agreement (BAA) with your organization covering PHI.

Your company’s specific guidance on compliant AI use (especially for integrated tools like Google Workspace’s Gemini) is paramount. Always, always, always prioritize client privacy and confidentiality.

Ready to unlock AI’s full potential? Let’s refine your prompting skills!

6 Principles for Mastering AI Prompts for Speech Therapy

To get the most out of AI, keep these essential principles in mind:

Effective AI prompts streamline your SLP workflow.

1. Clarity, Conciseness, & Specificity

Vague input equals generic output. The more targeted your request, the more tailored and useful the AI’s response.

Vague: “Give me therapy ideas.”

Specific: “Generate 5 engaging speech therapy activities for 7-year-olds working on initial /s/ blends, suitable for a 30-minute session, using a ‘superhero’ theme. List required materials for each.”

2. Assign a Persona (Role & Context)

Tell the AI who it is (e.g., “Act as an experienced pediatric SLP”) and provide relevant background. This is arguably the most crucial step! By assigning a role, you’re pulling from a specialized language subset, which drastically improves the quality and relevance of the output. This is key to effective SLP AI promts.

Generic: “Write a lesson plan.”

Contextualized: “Act as an experienced pediatric SLP working in a school setting. Create a 45-minute lesson plan for a small group of 2nd graders targeting narrative language skills, focusing on sequencing events and character emotions. Include a clear objective, suggested materials, step-by-step activities, and differentiation ideas.”

3. Define Output Format & Length

Don’t leave the output format to chance. Tell the AI exactly how you want the information organized (e.g., bullet points, table, short paragraph, email draft).

Unspecified: “Tell me about dysphagia.”

Formatted: “Summarize key differences between oral phase and pharyngeal phase dysphagia in a bulleted list, suitable for a 1-page handout for speech therapy graduate students.”

4. Specify Constraints & Exclusions

Tell the AI what not to include, or what limitations to follow. This helps filter out irrelevant or inappropriate information for your client’s needs.

General: “Give me articulation sentences for /l/.”

Constrained: “Generate 10 articulation drill sentences for initial /l/ sounds for a 4-year-old, but exclude any words related to food or animals, and ensure sentences are no more than 5 words long.”

5. Iterate & Refine (It’s a Conversation)

Prompting is rarely a one-shot deal. Think of it as a Socratic dialogue—start with a broader request, then refine the AI’s initial response with follow-up instructions until you hit perfection. This process of refining AI prompts for SLP output is essential.

Iteration Example:

You: “Give me goals for preschoolers.”

AI: (Provides very general goals)

You: “Okay, now make those goals SMART goals for receptive language, focusing on following 2-step directions.”

AI: (Provides more specific, measurable goals)

✨ Pro-Tip: Meta-Prompting (Make AI Write Your SLP Prompts)

If you are struggling to craft a detailed prompt, turn the tables and ask the AI to help you! This is a fast track to using all the principles above without having to think of every detail yourself.

Prompt to Refine Your Prompt (Ask for Clarification): Use this when you have a good idea, but aren’t sure what specific details the AI needs, or are unsure if you’ve thought of everything.

“You are an expert prompt engineer for speech-language pathology. I want to create a 30-minute therapy plan for a 5th grader working on figurative language. Based on this goal, ask me 5 questions to clarify what you need to write the perfect, detailed prompt.”

Prompt to Generate a Prompt: Use this to ensure your request includes all the necessary components (role, format, constraints).

“Act as an AI prompt expert. Write a detailed, effective prompt that will generate a handout for parents explaining the difference between an articulation disorder and a phonological disorder. The final prompt must include a specific role, a format, and a suggested reading level.”

6. Consider Tone & Audience

If you’re drafting communication for others, specify the desired tone and intended audience.

Prompt Example: “Draft a short email to a general education teacher explaining the process of a speech-language screening, using a friendly and informative tone, and inviting them to share any initial concerns.”

Create a 30-minute narrative therapy session for a group of three 3rd graders working on story grammar (characters, setting, problem, solution). Use a ‘detective’ theme. Include warm-up, main activity, and cool-down. Suggest print-and-go materials and a simple data collection sheet format.”

⚠️ The Guardrails: Where AI Falls Short for SLPs

Even with the best SLP AI prompt writing, AI is often confused by complex phonetic patterns or developmental nuances.

While AI is a powerful assistant, it’s crucial to understand where its current capabilities fall short. Remember that even with the most effective SLP AI prompts, your clinical judgment is irreplaceable.

Phonetics and Articulation Nuance: AI operates on text strings, not the underlying speech sounds, articulatory placements, or distinctive features. It can’t “hear” a velar fronting error. This means word lists for specific phonetic contexts (e.g., specific minimal pairs, /r/ contexts) often miss the mark. Always check its work!

Age/Grade Level Nuance for Language/Vocabulary: While AI can attempt to “simplify” language, its understanding of developmental language milestones, typical vocabulary acquisition, and the subtle nuances of age-appropriate content, syntax and semantics can be limited. Always review carefully for true developmental appropriateness.

Clinical Judgment Remains Yours: AI lacks clinical reasoning, empathy, and the ability to interpret non-verbal cues or individual client needs. Its output is based on patterns in the data it was trained on, not genuine understanding of a client. Your license, and your client’s well-being, is your responsibility.

As super-flexible powerhouses, SLPs routinely do the jobs of at least two people. What if you had a free assistant who could handle some of the heavy lifting? Enter AI tools for SLPs. Tools that could spark new ideas and streamline mundane tasks, so you could focus more on what you do best: transforming lives.

We’ve discussed privacy in Part 1, fact and myth in Part 2, and how AI works in Part 3. This post will give you some ideas on how to use AI to increase your productivity as an SLP. This isn’t about replacing your clinical expertise; it’s about equipping you with a powerful, ethical tool to turbocharge your efficiency and creativity.

Crucial Disclaimer: The Golden Rule of AI for SLPs

Before we dive into the practical applications, let’s establish the absolute, non-negotiable rule:

<img loading="lazy" decoding="async" width="1024" height="182" src="https://mrsspeechonline.com/wp-content/uploads/2025/09/AI-Golden-Rule-no-phi-1024x182.png" alt="A golden ruler with black markings.
NEVER input Protected Health Information (PHI) or any client-specific identifying data (such as names, baselines, unique characteristics, or highly specific details that could lead to identification) into general-purpose AI models like ChatGPT, Bard/Gemini’s public interfaces, or similar tools that do not have a specific Business Associate Agreement (BAA) with your organization covering PHI.

Your company’s specific guidance on compliant AI use (especially for integrated tools like Google Workspace’s Gemini) is paramount. Always, always, always prioritize client privacy and confidentiality.

With that vital principle in mind, let’s explore ethical and compliant ways AI can become your new favorite clinical assistant.

1. AI Can Supercharge Your Planning

A visual representation of how using AI in your speech therapy can save valuable time and improve therapy outcomes.

The blank page for a lesson plan or a new therapy goal can be daunting. AI in your SLP practice can be a brilliant brainstorming partner that helps you escape “therapy fatigue.”

Creating Lesson Plans & Activity Ideas: Need a therapy session around a specific theme (e.g., “outer space” for articulation, “community helpers” for social language)? Ask AI to brainstorm age-appropriate activities, vocabulary lists, or a rough session flow.

Prompt Example: “Brainstorm 5 engaging speech therapy activities for 7-year-olds working on /r/ production using an ‘ocean adventure’ theme.”

Developing Task Analyses: For complex skills (like tying shoes, ordering food, or writing a sentence), AI can help you break them down into discrete, manageable steps, providing a valuable framework for intervention.

Structuring Prompt Hierarchies: Design effective scaffolding for various tasks. AI can help you think through different levels of prompting from maximum to minimum support.

Generating Diverse Goal Wording Templates: Crafting clear, measurable, and relevant IEP or treatment goals is time-consuming. AI can provide various wording options for specific skill areas, which you can then adapt with your client’s unique, non-identifying data.

Prompt Example: “Provide 5 different ways to word a long-term goal for improving expressive vocabulary in a preschooler with language delay, focusing on measurable outcomes.”

2. Content Creation & Adaptation Made Easy with AI

<img loading="lazy" decoding="async" width="1024" height="1024" src="https://mrsspeechonline.com/wp-content/uploads/2025/09/slp-workflow-ai-process-adaptation-1024x1024.png" alt="A gold diagram featuring a gear with a person's silhouette at the center. The cyclical process shows how we constantly adapt materials to meet the diverse and unique student needs in our caseload.

Need to adjust existing materials or create new ones quickly? AI helps you differentiate and tailor content in seconds.

Adapting Existing Materials: Take a classic short story, news article, or educational text and ask AI to simplify it for a specific reading level (e.g., 3rd-grade equivalent) or to focus on particular concepts.

Prompt Example: “Simplify the plot of ‘The Three Little Pigs’ into language appropriate for a 1st-grade reading level, focusing on story elements.”

Generating Practice Items: Create endless lists of words, sentences, or short paragraphs targeting specific phonemes, grammatical structures, or vocabulary themes.

3. AI Documentation & Communication Shortcuts

This image represents the non-clinical writing tasks—emails, announcements, and general reports—that AI can help you polish and streamline.

While you should never input PHI into general AI, you can use it to polish and streamline all other written communications.

Proofreading and Grammar Checks: Use AI to review emails (that don’t contain PHI), general announcements, or non-confidential sections of reports for clarity and error reduction.

Summarizing Long Texts (General): Need to quickly grasp the main points of a professional article, a school policy document (without student info), or a long email thread? AI can provide a concise summary.

Rephrasing for Clarity: If you’re struggling to articulate a general idea or want to make instructions more straightforward for colleagues, AI can offer alternative, punchier phrasings.

A Practical Guide to Demystifying Generative Artificial Intelligence

Welcome back, fellow SLPs! In our last two posts, we tackled the crucial topic of AI and Clinical Data, as well as AI Truths & Myths. We learned that responsible AI use starts with safeguarding our clients’ privacy and understanding how AI works in practice.

We’ve debunked some common myths and explored the complex questions surrounding AI and data security. Now, let’s pull back the curtain and take a look at the mechanics room. In this post, we’ll demystify how AI tools actually work, providing a clear and simple explanation for SLPs.

Artificial intelligence can sometimes feel like magic, especially when it comes to generating coherent text for notes, therapy ideas, or reports. You type in a prompt, and seemingly out of nowhere, clinically relevant content appears. But behind this impressive capability lies a fascinating, albeit complex, system built on specific AI principles.

As SLPs, understanding the basics of how AI tools function is essential. It can not only alleviate some of the mystery, but also empower us to use these tools more effectively and with realistic expectations. So, let’s peek under the hood and demystify the mechanics of AI.

The Foundation: Large Language Models (LLMs)

To truly understand how AI works, you must first understand its base. At the heart of most AI tools you encounter that generate text are Large Language Models (LLMs). Think of these LLMs as incredibly advanced prediction machines for language.

They’re not thinking, feeling beings; instead, they are sophisticated algorithms. They are often built on a “transformer” architecture, that have learned to predict what word or sequence of words is most likely to come next. This ability to predict is the foundation of how they generate paragraphs, articles, notes, and even initial drafts of reports.

The Training Process: Learning from Text Patterns

The secret to this predictive power lies in the training process. LLMs are trained on truly massive amounts of text data – billions upon billions of words scraped from books, articles, websites, code, and just about any other digital text you can imagine. For models used in healthcare or clinical research, this could also include vast quantities of de-identified research papers, medical journals, and clinical guidelines.

During this training, the AI doesn’t process this raw text directly. Instead, it goes through a crucial step called tokenization.

What is Tokenization?

Tokenization is the process of breaking down the text into smaller, manageable units called tokens. These tokens can be whole words (“the,” “client”), parts of words (“-ing,” “re-“), or even individual characters or punctuation marks. The way text is tokenized can vary between different AI models, but the fundamental idea is to convert human language into a numerical format that the AI can process.

AI processes numerical tokens, not human thoughts.

Think of tokenization like the raw data stream in the movie The Matrix. Instead of seeing words and sentences, the AI sees a constant flow of these individual numerical tokens. This numerical representation is the fundamental language that the AI understands and manipulates.

Each unique token in the AI’s vast vocabulary is then assigned a numerical ID. Therefore, AI operates at this granular level of tokens and learns statistical relationships between them. It doesn’t typically “hold” or store entire books, articles, or clinical records as copies. Instead, it retains the learned patterns and probabilities of token sequences.

How is Tokenization Used?

Once the text is tokenized and converted into these numerical representations, the AI analyzes this vast ocean of language, identifying patterns. It learns the statistical probability of certain tokens appearing together. This includes the common structures of sentences and paragraphs, and typical ways humans express different ideas in various contexts (e.g., academic writing, informal speech, or clinical documentation).

For example, the AI learns that after the tokens “Client” followed by “demonstrated,” the token “progress” has a very high probability of appearing next in a clinical note. It learns the statistical relationships between diagnostic terms and common intervention strategies.

It’s all about recognizing these statistical relationships between these numerical tokens. It’s crucial to remember that the AI doesn’t “understand” these relationships in the way a human clinician does. It doesn’t grasp the clinical meaning, the client’s emotional state, or the unique context of a therapy session. It simply recognizes that, based on the numerical sequences it has processed, certain token sequences are more probable than others.

Mechanics of AI Text Generation: Prediction in Action

So, how does this learned knowledge translate into generated text that can assist an SLP? Let’s discuss the AI principles at work. First, you give an AI a prompt – a request, a question, or even just a few keywords (e.g., “Draft a SOAP note for a 5-year-old with articulation goals for /r/”). Then, the AI first tokenizes your input.

After that, it processes its learned patterns and the numerical IDs of these tokens. It then predicts the most likely next token (represented by its numerical ID). This prediction is heavily influenced by the parameters of the LLM. Think of these parameters, often billions or even trillions, as the internal “knobs and dials”, like the Oz Wizard. The AI adjusts these during its training to learn the strength of the statistical relationships between tokens. The more parameters an AI has, generally the more nuanced and sophisticated its predictions can be.

The Output of the AI Machine

The AI takes that newly predicted token. It compares the original prompt and the token it just produced. Then it predicts the next most likely token, again relying on its vast network of parameters. This process continues, token by token, until the AI determines it has reached a stopping point. Finally, these numerical tokens are converted back into human-readable text.

Think of it like a super-powered version of the autocomplete feature you might use when typing. However, it operates at the level of fundamental text units, and is guided by an incredibly complex web of learned relationships stored within its parameters. The “creativity” or “clinical relevance” we sometimes see in AI-generated text isn’t true clinical reasoning in the human sense. Rather, it is the result of the sheer vastness of the data it has been trained on. It has learned the intricate statistical relationships between these tokens. It is all governed by its numerous parameters.

What AI Isn’t: Understanding the Clinical Limitations

Finally, it’s vital to reiterate what AI isn’t, especially from an SLP perspective. Even understanding the mechanics of AI, its human-like responses can often muddy the waters.

Knowing the limits of AI is essential.

It isn’t conscious. It doesn’t have emotions, personal experiences, or clinical judgment to draw upon.

It cannot understand or provide the nuances of human communication. Sarcasm, irony, or the subtle emotional context of a client’s utterance are often difficult for AI to understand. They are even harder for AI to create (don’t ask it to generate jokes).

It is limited in understanding highly nuanced linguistic elements like prosody, intonation, or specific phonetic realizations (e.g., truly hearing and analyzing a distorted /s/ or a lateral lisp). These areas are critical for SLP assessment and intervention, but challenging for current AI to grasp fully and accurately.

It cannot build genuine therapeutic rapport or provide the empathetic connection essential for effective therapy.

It cannot make a differential diagnosis or independently formulate a comprehensive, individualized treatment plan based on dynamic clinical interactions.

AI can’t handle very large chunks of information. This is a significant limitation to remember, especially when working with long documents. More advanced and often paid AI versions, which are hopefully HIPAA-compliant, can process larger numbers of tokens.

Its output, however impressive, is ultimately based on the patterns it has learned from the text it has been trained on. You must remember that it is processed at the level of tokens and statistical probabilities, not genuine clinical understanding. You MUST always double-check output for accuracy, completeness, and bias!

Looking Ahead: Understanding AI as Your SLP’s Toolbox

Understanding the core mechanics of AI—that it’s a sophisticated text prediction machine built on statistical patterns—is the key to unlocking its potential without falling for the hype. This knowledge empowers us to see AI not as a magical entity, but as a powerful tool in our clinical process. It’s an assistant that can handle routine tasks, potentially allowing us to save precious time and redirect our energy to where it matters most: our clients.

The world of speech-language pathology (SLP) is transforming alongside the rise of artificial intelligence (AI). AI isn’t here to replace your expertise; instead, it can be a powerful tool to enhance your clinical practice and empower your clients. Let’s explore how AI can become your trusted partner in speech therapy:

Supercharge Therapy Planning:

Personalized Materials: Struggling to find the perfect stimuli for a particular client? AI can generate engaging stories, activities, and exercises tailored to their specific needs and target sounds.

Brainstorming Buddy: Facing a complex case? Use AI to brainstorm creative therapy approaches and explore different treatment options. Here, your expertise is crucial in selecting the most appropriate options based on your client’s individual needs and learning style.

Boost Client Engagement:

Interactive Activities: AI can create interactive games and exercises that make therapy sessions more fun and motivating for clients, especially younger children. However, use your expert insight to ensure the difficulty level and format of the activity are appropriate for your client’s abilities.

Real-Time Feedback: Imagine having an AI assistant that provides real-time feedback on a client’s pronunciation, intonation, or fluency! This can offer valuable insights and promote self-monitoring skills. Remember to analyze the AI’s feedback alongside your own observations to provide the most comprehensive guidance to your client.

Revolutionize Documentation:

Automated Note Taking: Free yourself from tedious paperwork! AI tools can transcribe therapy sessions and generate draft session notes based on the audio recording. You can then review and edit them for accuracy, incorporating your clinical observations and insights.

Speech and Language Sampling: Automatic Speech Recognition (ASR) tools can transcribe the recordings with high accuracy, saving SLPs valuable time. Additionally, AI can analyze the transcribed speech samples for various aspects. It can identify features like articulation errors, fluency patterns, and vocabulary usage. This should complement your human expertise, which remains crucial in interpreting the data.

Ethical Considerations:

Focus on Collaboration: Remember, AI is a tool to enhance your practice, not replace your specialized skills. Use AI-generated materials as a starting point, then personalize them to fit your client’s specific needs.

Data Privacy: Ensure any AI tools you use comply with data privacy regulations and handle client information securely. I’d recommend leaving out names or any identifying information.

Prompting Your AI Assistant Strategically:

To get the most out of AI tools, it’s important to provide clear and concise prompts that incorporate your expertise:

Client Profile: Briefly describe your client’s age, communication goals, specific challenges they are facing, and their learning style. This information helps the AI tailor its output to your client’s needs.

Target Area: Specify the area you’d like AI to assist with, such as creating articulation exercises (mentioning the specific sounds you’re targeting), developing social communication activities (specifying the desired social skill), or crafting reading comprehension worksheets (indicating the reading level and comprehension skill you want to focus on).

Desired Style: Indicate any preferences you have for the AI’s output. Do you want the activity to be interactive, story-based, or game-like? Consider your client’s interests and learning style when making this choice.

Remember: You are the expert.

Use your professional knowledge to evaluate and adapt any AI-generated materials to ensure they are appropriate for your client’s specific needs and goals. The AI is a tool to empower you, not replace you.

Ready to Get Started?

Explore the world of AI resources available for SLPs! Many AI writing and speech recognition tools are readily available. Start by identifying specific areas in your practice where AI could be most beneficial.

By embracing AI as a collaborative partner, While keeping your professional knowledge at the forefront, you can streamline your workflow, personalize your therapy approach, and ultimately empower your clients to achieve their communication goals.

Envision this:

Sarah, a young client, struggles with sharing toys during playtime. You, the SLP, identify the need to create a social story to help Sarah understand the concept of sharing and taking turns.

Here’s where AI becomes your partner:

Prompting the AI:

The SLP prompts the AI with the following information:

Target Social Skill: Sharing toys

Character: A young girl named Sarah

Desired Tone: Positive and encouraging

AI Generates Story:

Sarah loves playing with her toys! She has a cuddly teddy bear, a bright red car, and a set of colorful blocks. One sunny afternoon, Sarah is playing in the park when she sees another child, Ben, sitting alone. Ben looks a little shy.

Sarah walks over to Ben with a big smile. "Hi Ben! What are you playing with?" she asks.

Ben mumbles something about having a toy truck but not wanting to play alone.

Sarah has an idea! "Would you like to play with my blocks?" she asks.

Ben's eyes widen. "Really? Can I?"

"Sure!" says Sarah. "We can build a big tower together! Do you want to put the red block on top?"

Ben smiles and carefully places the red block on top of the growing tower. They take turns adding blocks, giggling as the tower gets taller and taller.

Suddenly, another child, Emily, walks over and looks at their tower. "Wow! That's amazing!" she exclaims.

"Would you like to help us build even higher?" Sarah asks Emily.

Emily joins in the fun, carefully adding more blocks to the tower. They all work together, taking turns and laughing as they build a magnificent tower of blocks.

SLP’s Integration:

After reviewing the story to insure it is appropriate to the child, the SLP reads the story with Sarah, pointing out the pictures (ideally featuring a character resembling Sarah and toys she recognizes) and discussing the emotions of the characters. They talk about how Sarah shared her toys with Ben and Emily, and how everyone had more fun playing together.

Personalizing the Story:

The SLP notices Sarah particularly loves animals and edits the story to mention her favorite stuffed animal, Mr. Snuggles, instead of a teddy bear. They can further personalize the story by adding details about the park (mentioning any specific equipment Sarah enjoys) or the type of tower they build (e.g., an animal tower if Sarah is interested in animals). Please be aware: personalization like this could constitute Personal Health Information under HIPAA. Do not share PHI in ANY AI platform without checking that it is HIPAA compliant! See more in AI & SLPs Series, Part 1: Clinical Data Privacy.

The SLP can also create follow-up activities based on the story, such as role-playing sharing scenarios or drawing pictures about taking turns using their favorite toys.

Benefits of AI in Social Story Creation:

Overcomes Writer’s Block: AI can provide a starting point or inspiration for the SLP.

Saves Time: AI can generate a basic story outline, allowing the SLP to focus on customization.

Engaging Language: AI can create stories with a child-friendly tone and vocabulary.

This is just one example of how AI can be used as a tool to enhance the work of SLPs. Remember, the SLP’s professional knowledge and experience remain crucial in tailoring the story to the specific needs of each client.

Yearly Initiatives: Transform Your Speech Therapy Sessions!

Feeling overwhelmed by IEP meetings and juggling multiple speech therapy goals? You’re not alone! This blog post dives deep into the world of strategic planning. It offers tips and tricks to maximize student progress and transform your therapy sessions. Here, we’ll explore the concept of yearly initiatives. This powerful approach can streamline your planning and supercharge your students’ learning.

Streamlined Goal Setting with Yearly Initiatives

Traditionally, we’ve used cyclical approaches for phonological processes. But have you considered extending this idea to encompass broader language development? This post explores the magic of yearly goal initiatives. It involves choosing a specific language area you focus on for the majority of your caseload throughout the year.

Here’s why yearly initiatives are a game-changer:

This graphic symbolizes focusing on a central language area for year-long therapy, allowing for deeper learning and more impactful outcomes. © 2025 Mrs. Speech LLC.

Deeper Dives: Focus on crucial areas like problem-solving, inferencing, prefixes/suffixes, defining/describing, or comprehension instead of broad or multiple themes. This allows for a deeper dive and more impactful therapy.

Cohesive Learning Journey: Focusing on one area creates a cohesive and effective learning journey for your students. This ensures all activities build upon each other.

Research-Backed Benefits: Studies on spaced repetition show that distributing learning over time with focused practice sessions leads to better long-term retention.1 This aligns perfectly with yearly initiatives. Students engage with a key skill throughout the year, solidifying their understanding and building strong memory pathways.

Myelination Matters: Research on myelination suggests that repeated practice strengthens the connections involved in a skill.2 The focused sessions encouraged by yearly initiatives can promote myelination. This increase the speed and efficiency of processing the targeted skill.

Individualization Within the Initiative

While your yearly initiative guides your planning, you can certainly individualize goals within it. This might include:

This illustration of a pyramid of children symbolizes how unique student needs are supported within a shared yearly therapy initiative.

Adjusting difficulty levels (e.g., percentage accuracy, task complexity)

Tailoring prompting strategies (frequency, type)

Shifting focus (utilizing similar materials for other purposes)

Utilizing individualized rubrics for assessment

Example: Let’s say your yearly initiative is problem-solving.

Joe: Focuses on identifying problems in longer stories, creating multiple solutions, and predicting outcomes.

Jane: Needs visual aids and shorter stories to identify problems and may require prompting to create solutions. Uses the same rubric as Joe.

Carl: Works on explaining solutions with appropriate pragmatics and grammar. Also provides a model for higher level problem-solving for the other students.

Sample Yearly Initiatives:

Problem-Solving (social and safety scenarios, sequencing, predictions, wh-questions)

Inferencing & Context Clues (predictions, character traits, vocabulary, multiple meaning words, non-literal language)

Prefixes & Suffixes (vocabulary building, context clues)

Defining & Describing (categories, antonyms, synonyms, multiple meaning words, inferencing, forming wh- questions, vocabulary)

Comprehension (wh- questions, main idea, summarizing, story arc, sequencing)

Finding the Perfect Balance: Goals and Efficiency

Now, let’s talk about IEP goals. Most language students require at least two therapy goals. However, bombarding them with too many can be counterproductive. This aligns with Cognitive Load Theory, which suggests that overwhelming working memory with excessive information or tasks impedes effective learning.3

Here’s a good rule of thumb: Limit to 2 or fewer goals per 30-minute/week on the IEP, unless the skills are quick to learn or complement each other (e.g., identifying story elements and summarizing).

5 Goals is the High End: Prioritize focus! With fewer goals, progress accelerates. You can always add more goals later if students master their current ones.

The Power of Focused Sessions: Deep Learning vs. Superficial Exposure

Imagine an orchestra. Are you more likely to learn the music if you spend 5 minutes on each song, or dedicate focused sessions to mastering one at a time? The same principle applies to language therapy.

Just as orchestra musicians master their parts through focused practice, dedicating consistent time to a single goal leads to deep learning in language therapy.

Deep Learning: By dedicating 3-6 sessions (1-3 weeks) to a single goal, you allow students to truly grasp the concept. They build strong memory pathways.

Data Collection Made Easy: This focused approach simplifies data collection. Collect data at the beginning, end, and potentially during the middle of a longer cycle. This helps document progress and regression (critical for ESY services). Create a rubric or data probe usable throughout the year for consistent data collection across multiple students.

Elaboration for Long-Term Learning: By dedicating 3-6 sessions (1-3 weeks) to a single goal, you allow students to truly grasp the concept and build strong memory pathways. This helps them apply their learning to different contexts. This deeper understanding, fostered by elaboration, is crucial for long-term skill development.4

A Personal Discovery: The Unexpected Benefits

This concept wasn’t something I initially planned. In grad school, I was taught to cover each goal every session. It left me feeling overwhelmed! About 10 years ago, after taking a course on executive functioning, I noticed many of my students struggled with these crucial skills. Naturally, the focus of their IEP goals shifted to address these deficits.

The unexpected result? A year-long focus on problem-solving! This unintentional initiative highlighted the importance of these skills and created a cohesive therapy journey for my students. All activities built upon each other, addressing their specific needs. Interestingly, the collaborative environment fostered unexpected benefits. Students seemed to learn from each other’s problem-solving strategies, creating a more engaging atmosphere.

Planning Efficiency and Deeper Learning: A Win-Win

Just as a bullet train’s design allows for maximum efficiency, yearly initiatives streamline planning and supercharge learning for deeper impact in therapy.

The impact wasn’t limited to student learning. My own planning became much more efficient. Therapy materials were readily available, student goals were clear, and data collection was streamlined. We weren’t constantly switching gears – we were all focused on mastering the same core skill, allowing for deeper learning and richer student interaction.

This experience truly solidified the power of yearly initiatives in my therapy sessions. It’s a framework I wholeheartedly recommend to any speech therapist looking to maximize student progress, lesson planning, and create a more cohesive learning journey.

But It’s Not Perfect: Considerations for Yearly Initiatives

While I’ve witnessed the positive impact firsthand, and some research supports this approach, there are limitations to consider:

Limited Research: While studies on spaced repetition and myelination offer some support, more research is needed to definitively establish yearly initiatives as “evidence-based.”

Individualized Needs: Yearly initiatives may not be suitable for every child on your caseload. Consider developing two initiatives targeting different areas of language for diverse needs.

Stimulus Material: Because you’re focusing on the same skill, you’ll need a variety of engaging activities and materials to keep things fresh for students. The upfront investment in creating these materials may be higher than with a constantly rotating set of goals.

Medicaid and District/State Requirements: If you are required to address every goal or need percentages to report for every session, you may have to revise or take time out of the initiative cycle to fulfill these requirements.

Despite these considerations, yearly initiatives offer a powerful framework for planning and delivering effective speech therapy. By incorporating these tips and acknowledging the limitations, you can explore how yearly initiatives can benefit your students and streamline your practice.

Decoding Documentation: Your Guide to Session Notes

(Plus a Free Resource!)

As Speech-Language Pathologists, we wear many hats – therapist, educator, data analyst, and yes, meticulous documenter. When it comes to Medicaid and other payors, thorough and accurate session notes aren’t just best practice; they’re essential for reimbursement and demonstrating the value of our services. Navigating the specifics of what to include can feel overwhelming. Hopefully, breaking it down into key components with this essential documentation guide makes the process much more manageable.

This post will walk you through the critical elements of session-specific documentation, ensuring you’re covering all your bases and feeling confident in your record-keeping. Plus, I’m excited to share a free resource to help streamline this process!

The Foundation: Session-Specific Information

Building strong, clear session notes block by block.

Each therapy session note should act as a clear snapshot of what occurred. Reviewers need to understand the what, how, and why behind your interventions. Here’s a breakdown of the key areas to address:

Brief Summary of the Session’s Focus/Goal(s)

Start by clearly stating the primary objective of the session. This should directly link back to the client’s Individualized Education Program (IEP) goals (if applicable) or Plan of Care (POC).

Example: “Focused on improving production of /r/ in initial position at the word level (IEP Goal #2).”

Specific Activities and Techniques Used

Detail the therapeutic interventions you employed. Be precise enough that another SLP could understand and potentially replicate your approach.

Client’s Response to Therapy

Document how the client engaged with the activities and their overall participation. Note both successes and challenges.

Examples:

“Client produced target /r/ sounds with 70% accuracy with verbal models.”

“Demonstrated increased comprehension of two-step directions when paired with visual cues.”

“Required frequent redirection to maintain focus during the tabletop activity.”

“Actively participated in the barrier game, taking turns and providing clear instructions.”

Data Collected (if applicable)

If you gathered specific data during the session (e.g., percentages, frequencies), record it here. Objective data strengthens your documentation of progress.

Example: “Collected data on the client’s ability to answer ‘who,’ ‘what,’ and ‘where’ questions related to a short story, achieving 85% accuracy.”

Any Modifications or Adjustments Made to the Intervention

Note any deviations from your planned activities based on the client’s immediate needs or responses. This demonstrates your clinical decision-making.

Example: “Due to the client’s increased frustration with word-level tasks, the session was modified to focus on sound identification activities for /r/.”

Home Program/Carryover Activities (if assigned)

If you provided the client or caregiver with tasks to work on outside of the session, clearly outline them.

Example: “Provided the caregiver with a list of target /r/ words to practice at home using a game-based approach.”

Demonstrating Progress Towards Goals

<img loading="lazy" decoding="async" width="1280" height="775" src="https://mrsspeechonline.com/wp-content/uploads/2025/07/woman-4431531_1280.webp" alt="A digital illustration of a professional woman in a business suit holding a laptop.
Your session notes should clearly link back to the client’s overarching goals.

Brief Statement of Progress (or lack thereof) Towards Current Goals: Summarize the client’s progress in relation to their IEP/POC goals based on the session’s activities and their performance.

Example: “Demonstrated initial progress towards IEP Goal #2 (production of /r/) with improved accuracy in structured tasks with models.”

Objective Measures of Progress (when possible): Reference any data collected that supports your progress statement.

Example: “The 70% accuracy achieved on target /r/ words represents a 15% increase from the previous session’s baseline data.”

Other Essential Documentation Information

Don’t forget these crucial details:

Any Communication with Caregivers, Teachers, or Other Professionals: Document any significant conversations related to the session or the client’s overall progress.

Example: “Spoke with the classroom teacher regarding the client’s carryover of sentence structures in academic tasks.”

Signature and Professional Title: Your signature verifies the accuracy of your documentation.

Beyond the Essential Documentation Guide: Key Considerations

Keep these points in mind for Medicaid documentation:

Clarity and Conciseness: Use straightforward, professional language. Avoid excessive jargon or unclear abbreviations.

Objectivity: Focus on observable facts and data rather than personal opinions.

Accuracy: Ensure all information is truthful and reflects the services provided.

Completeness: Include all the required elements for each session to avoid claim denials.

Timeliness: Document sessions promptly to maintain accuracy and facilitate efficient billing.

What is up with AI?

The buzz around artificial intelligence (AI) has hit pretty much every corner of life these days. Speech-language pathology (SLP) is no different. For us clinicians, these new AI tools have brought a real mix of feelings. There’s certainly curiosity, but also a good chunk of “Wait, what?” Maybe you feel some apprehension and, yes, let’s be honest, even some straight-up fear. How do we even begin to separate AI Truth vs AI Myth?

Ads scream about AI writing full reports in minutes, and professional communities discuss blanket policies on AI use. It’s easy to get caught up in a swirl of misunderstandings about what this technology actually is, and what it means for SLPs.

This is the second of an 8-part weekly series on AI. See the bottom of this post for a full list of planned posts in this series. Each post ends with a quick 3-4 question poll that will be reported in the final part.

In this post, we’re going to cut through all the noise and tackle some of the most common myths floating around about AI and clinical practice. Our goal is to clarify AI’s true capabilities and limitations. I aim to help you navigate this changing scene with solid information and perspectives for informed decision-making.

AI Truth or Myth 1: AI will replace human SLPs.

Myth Debunked – This is probably the biggest worry. Will AI make human speech-language pathologists a thing of the past?

The truth is, AI can generate text, analyze patterns, and even provide basic feedback. However, there is a fundamental difference between artificial intelligence and human clinical expertise.

Our work as SLPs often comes from our own lived experiences, deep empathy, unique ways of understanding human communication. We strive to foster growth and connection in complex individuals in an even more complex society.

AI, as it stands, simply doesn’t possess that. It operates by identifying patterns from vast amounts of data, but it lacks genuine thoughts, emotions, or lived experiences to draw upon. That nuanced assessment, the understanding of client needs and motivations, and truly original, resonant therapy approaches? That’s human SLP territory, and it’s irreplaceable.

AI Truth or Myth 2: Using AI goes against HIPAA and client confidentiality.

Myth Debunked – This is a paramount concern for SLPs, and rightly so. The myth suggests that any use of AI tools inherently breaches client privacy regulations like HIPAA. It’s true that using public, unsecured AI models with protected health information (PHI) would be a severe violation. However, the reality is far more nuanced.

Responsible and ethical AI integration within an SLP practice is entirely possible. It involves understanding and utilizing secure, HIPAA-compliant AI tools. You must fully de-identifying data before inputting it into less secure models, and always adhere to established privacy protocols. This series will delve deeper into safeguarding client data. For now, understand that the myth of blanket violation is a misrepresentation.

AI Truth or Myth 3: AI use is unprofessional or inherently unethical for SLPs.

Myth Debunked – Viewing AI as a compromise of professional standards overlooks its significant potential as a powerful tool. Think of it like a massive clinical resource. It’s an advanced documentation assistant, or a brainstorming partner. These are resources SLPs have utilized for ages to enhance their work.

When used in a responsible and transparent way, AI can assist with many clinical tasks. It’s adept at brainstorming therapy ideas, drafting session notes, creating client education materials, or summarizing research. The crucial difference lies in how you use it.

Attempting to pass off AI-written material as your own unreviewed clinical work is certainly unethical. However, using AI to augment your own skills, streamline certain tasks, or provide initial drafts can actually be very beneficial. It can allow you more time for direct client interaction and clinical reasoning.

AI Truth or Myth 4: AI-generated clinical content (e.g., notes, therapy plans, materials) is undependable or inaccurate.

Myth Confirmed – Thinking AI can just instantly produce perfect, clinically sound content without any effort? That’s not truly how it works. AI can certainly assemble grammatically correct sentences for notes or even mimic certain therapy activity styles. What it often lacks is genuine clinical depth, a sense of individualized client needs, and that unique insight that makes therapy truly effective.

AI-generated text can sometimes sound generic, repeat itself, or even introduce clinical inaccuracies or inappropriate suggestions (called “hallucinations”). Fact-checking, personalization, and adaptation to ensure the materials are truly appropriate, accurate, and ready for use are critical. AI is a drafting assistant, not a final clinical decision-maker.

AI Truth or Myth 5: AI can provide definitive diagnoses or independently determine treatment plans.

Myth Debunked – This is a critical misconception. AI models can process vast amounts of medical and clinical data, identify patterns, and flag potential concerns. However, they cannot perform differential diagnoses or independently create comprehensive, individualized treatment plans. AI lacks clinical reasoning and cannot integrate diverse assessment data from multiple modalities. It also lacks nuanced human behavior understanding, missing the ethical responsibility required for core SLP functions.

AI can be a powerful support tool for clinicians. It can summarize research, analyze speech samples for patterns, or brainstorm intervention strategies. It’s critical to remember that the SLP remains solely responsible for all diagnostic conclusions, treatment planning, and ultimate clinical decisions.

AI Truth or Myth 6: AI-powered tools will make therapy impersonal and reduce human connection.

Myth Debunked – The fear that technology will diminish the crucial human element of therapy is understandable. However, this myth overlooks how AI can actually enhance human connection. The truth is, by automating or streamlining repetitive administrative and preparatory tasks, AI can free up valuable time.

This freed-up time means SLPs can dedicate more energy to direct client interaction, building rapport, engaging in personalized therapy activities, and providing the deeply empathetic support that defines our profession. Therefore, AI becomes a tool to optimize our time, allowing us to be more present and connected with our clients, not less.

AI Truth or Myth 7: AI can perfectly understand and replicate human speech, including nuances like accent, dialect, or prosody.

Myth Debunked – Automatic Speech Recognition (ASR) technology has made incredible strides. However, the myth of its universal flawlessness is untrue. It is not unbiased for all populations.

Current AI models can struggle with diverse accents, non-standard dialects, and speech sound disorders. Subtle but critical emotional and linguistic nuances like sarcasm, intonation, or pitch variations are also frequently missed.

For SLPs, relying solely on flawed ASR for diagnostic or therapeutic purposes could lead to misinterpretations, inaccurate assessments, or inappropriate intervention strategies. This is especially true when working with culturally and linguistically diverse populations. The SLP’s ear and clinical expertise remain paramount for accurate phonetic, phonological, and prosodic analysis.

AI Truth or Myth 8: Using AI requires extensive tech expertise or coding skills.

Myth Debunked – Many SLPs may feel intimidated by AI, believing it demands a deep understanding of computer science or programming. The myth suggests that only tech-savvy individuals can leverage AI in their practice. The reality, however, is that most common generative AI tools are designed with user-friendly interfaces that resemble simple chat programs.

You interact with them using natural language prompts, much like you would talk to a colleague. While understanding how to prompt effectively is key (a topic we’ll cover later in this series!), it requires no coding skills whatsoever. This accessibility makes AI a tool that can be integrated by any SLP, regardless of their tech background.

AI Truth or Myth 9: AI is only beneficial for a small niche of SLP practice (e.g., articulation or language).

Myth Debunked – The misconception here is that AI’s utility is narrowly confined to specific areas of communication disorders. In reality, AI’s potential applications span the vast breadth of SLP practice. Beyond generating articulation drills or language activities, AI can assist in areas such as:

Social communication: Brainstorming social scripts or narrative scenarios.

Fluency: Organizing research or drafting educational materials on disfluencies.

Voice: Summarizing literature on vocal hygiene or voice therapy techniques.

Dysphagia: Assisting with literature reviews or creating patient education handouts.

AAC: Generating ideas for symbol sets or communication board layouts.

Professional Development: Summarizing research articles or creating CEU presentation outlines.

AI is a versatile assistant that can be adapted to enhance efficiency and creativity across virtually all domains of speech-language pathology.

Moving Forward with Clarity and Facts:

It’s crucially important for us SLPs to gain a clear understanding of what AI can and can’t do right now. By busting these common myths, we can begin to see AI is not something that’s going to take our jobs. Instead, it is a tool that might actually prove helpful in our clinical process, allowing us to spend more time where it matters most: directly with our clients.
