As an SLP, parents and teachers are often concerned about middle school language development. They often ask me about a student who just doesn’t seem to “get it.” They’ll describe a child who takes everything literally, misses a joke, or seems to lack empathy. Since these kids are fluent speakers who sound like adults, it’s easy to assume these behaviors are a sign of a problem.

This is the middle school paradox. We assume that since their language sounds mature, their brains are fully equipped to handle complex social and communication skills. But what the lack or immaturity of these skills wasn’t a sign of a delay, but evidence of a developing brain? The truth is, while middle schoolers are great at sounding mature, their ability to handle the subtle, unspoken parts of communication is still a work in progress.

The Middle School Brain is Still Developing

It might seem they’ve suddenly grown up, but a middle schooler’s brain is still under construction. The prefrontal cortex, which handles complex thinking and self-control, won’t be fully developed until they are in their mid-twenties. This means many of the skills we adults take for granted are still being built. And honestly, don’t we all struggle with some of these things ourselves, no matter our age?

Understanding Abstract Language

Think about sarcasm, idioms, or metaphors. When a teacher says, “Oh, that’s just what I needed,” after a student drops papers everywhere, most adults get the joke. A middle schooler might not. Understanding this requires going beyond the literal words and reading the person’s tone and context.

This is also where inferencing comes in—it’s the ability to “read between the lines” and figure out what’s not explicitly stated. While a student might be able to define idioms on a school test, using and understanding them in a fast-paced, real-life conversation is much harder. Understanding colorful phrases is a still developing middle school language skill.

Navigating Social Cues

This is more than just empathy—it’s the ability to see things from another person’s point of view. Middle schoolers are so focused on figuring out their own identity that they can seem self-centered. It’s not a lack of caring; it’s a natural part of their social language development in middle school. A person has to understand their own self before they can truly understand others.

Part of this age’s awkwardness is that as they try to figure out their own identity, middle schoolers often “try on” different attitudes, styles, and interests. A student who is quiet one week might be outspoken the next. This isn’t being fake; it’s a natural form of social experimentation as they explore different versions of themselves.

Handling Big Emotions

Middle schoolers understand basic feelings like happy or sad, but they often struggle to name more complex emotions like frustration or anxiety. This can lead to them acting out because they lack the vocabulary to express what they’re truly feeling.

They might feel a physical response—a tight stomach, a pounding heart—without knowing what emotion is causing it. They’re also still developing the self-awareness and self-talk needed to pay attention to these body signals before they become too overwhelming.

More Skills Developing in Middle School Brains

A few other language skills are still growing in middle schoolers, affecting their ability to learn and succeed.

Vocabulary and Word Knowledge

It’s not just about learning new words, but understanding all the different meanings and contexts for a single word. For example, a student might know that “trunk” refers to a suitcase, but they may not yet understand its use in phrases like “elephant trunk” or “tree trunk.”

Complex Language Comprehension

While they are using more sophisticated language, they can still struggle to fully understand long, grammatically intricate sentences in academic texts. The ability to break down and process this kind of intricate language is a skill that develops with exposure and explicit instruction.

Memory and Metacognitive Strategies

Middle schoolers are not only learning new things, but they are also beginning to learn how they learn. They’re developing metacognition—the ability to think about their own thinking. This includes learning and applying strategies for memory, such as using mnemonic devices or chunking information to make it easier to remember.

How You Can Help Middle School Language Development

Your support is the scaffolding they need to build these crucial skills. Here’s how you can help:

• Don’t Just Clarify, Explain. If you use sarcasm, follow up by saying, “I’m just being silly; I actually didn’t need that.” This helps them learn to spot a joke.
• Keep It Simple During High Emotion. When a middle schooler is upset, their thinking brain is offline. Use minimal words and direct instructions, like “Sit down. Breathe.”
• Talk It Through. Help them reflect on social situations. Ask, “Your friend looked sad. Why do you think that is?” This helps them practice perspective-taking and gives them the language to identify complex emotions.

Their challenging behaviors are often a sign of a developing brain, not a lack of effort or intelligence. By viewing them as a work in progress, we can provide the guidance they need to become capable and confident adults.

A Final Thought: The Power of Perspective

Middle school is a time of immense change, and much of the behavior that adults find confusing or frustrating is simply a reflection of a brain that is in an incredible, albeit awkward, state of growth. By reframing our perspective, we can stop seeing challenging behaviors as defiance. We can start viewing them, not only as naturally developing middle school language skills, but also as opportunities for teaching and connection.

Picture this: You’ve been teaching idioms non-stop, patting yourself on the back because your students just learned ‘Spill the beans.’ Moments later, they’re on TikTok asking ‘It’s giving what?’ or saying ‘No cap.’ Are we truly equipping them for the language they live in?

While I love the idioms from yesteryear, such as all the lovely sayings from Aesop’s fables, you don’t hear them much in everyday life. The majority of students haven’t even been exposed to Aesop’s fables beyond a story or two.

Try listening in at a restaurant some time, especially to a group of younger people, or reading song lyrics from a popular new song. I imagine the older you are, the less you’ll recognize their figurative language. If they don’t hear it in their lives, why are we pushing it? Have you ever asked them “Have you heard this before?” I have, and I usually get a baffled shake of the head.

Language is a living, breathing thing. It’s constantly evolving, shedding old phrases and creating new ones. We sometimes have to let go of the old and accept the new. That means it’s time to ask a crucial question: Why are we even bothering with teaching idioms in the first place?

The True “Why”: Understanding Non-Literal Language, Not Memorizing a Secret Code

When we teach idioms to students, our primary goal shouldn’t be to have them memorize a fixed list of phrases as if they were secret codes for an exclusive club. (Honestly, whose club would they be codes to? Grandma’s bridge club?) If our aim is simply rote memorization, we’re missing the point entirely.

The real “why” behind idiom instruction is far more profound: to foster a robust understanding of non-literal language.

Benefits of Understanding Non-Literal Language

• Inferencing and Abstract Thinking: Understanding idioms requires moving beyond the concrete and literal. It builds critical thinking skills necessary for comprehending metaphors, sarcasm, humor, and complex social cues.
• Social Communication: So much of human interaction relies on implied meanings and figurative speech. A student who struggles with non-literal language might miss jokes, misinterpret intentions, or feel perpetually “out of the loop.”
• Reading Comprehension: Texts, even those for younger readers, are rife with figurative language. A strong grasp of non-literal concepts directly impacts reading comprehension and overall academic success.
• Language Flexibility: Understanding that words can have multiple layers of meaning makes students more agile and adaptable language users.

If our students are simply memorizing that ‘raining cats and dogs’ means heavy rain but don’t grasp the underlying concept that words can take on completely new meanings when combined, or how to figure them out, we’re not truly serving them. We’re giving them a fish for today rather than teaching them how to fish for a lifetime of language.

Why ‘Grandma’s Idioms’ Aren’t Cutting It Anymore

The challenge with focusing exclusively on teaching traditional idioms is their diminishing relevance. While culturally significant, phrases like ‘kick the bucket’ or ‘let the cat out of the bag’ are rarely encountered in the daily conversations, social media feeds, or digital interactions of today’s youth.

When we present students with idioms that feel foreign and disconnected from their world, we risk:

• Demotivation: Learning feels like an arbitrary chore rather than a relevant skill.
• Lack of Context: Without real-world exposure, the idioms lack natural context, making them harder to internalize and generalize.
• Inefficiency: Precious therapy and classroom time are spent on vocabulary that may never be actively used or encountered.

Of course, the classics still have their place. Learning to decode figurative language is a timeless skill. Consider exploring a time when language and culture were in a period of change, much like today.

The Solution to Understanding Figurative Language

Instead of teaching a list of static idioms, let’s empower our students, working alongside teachers and parents, to become idiom detectives! The goal is to identify, interpret, and understand the non-literal language they encounter every single day.

I propose creating a Living Idiom Dictionary—a dynamic, collaborative resource that evolves with the language our students actually use and hear. My Living Dictionary for Slang and Idioms is a free product that can help you get started.

How it Works:

• Spot It! Encourage students to listen for phrases they don’t understand, especially in casual conversations, TV shows, movies, social media, song lyrics, and video games.
• Capture It! When an unfamiliar phrase pops up, jot it down. This can be a physical notebook, a shared digital document, a whiteboard in the classroom, or a note on the fridge at home.

• Define It (Collaboratively)! This is the crucial step. Instead of just looking it up, discuss:What do the individual words mean literally?
  • What do you think it means in this context?
  • Where did you hear it? Who uses it?
  • Work together (SLP/teacher/parent with student, or even peers) to figure out the non-literal meaning. Sometimes, a quick online search might be necessary to confirm.
• Contextualize It! Write down the idiom, its non-literal meaning, and an example sentence of how it’s used. Bonus points for drawing a literal and a figurative picture! Teaching How to Decode Figurative Language

You can use my free Figurative Language Decoding Visual Aid to help students with the process of untangling the meaning of figurative language. This simple, two-page guide includes a figurative language decision tree and a 3-step decoding guide, turning students into language detectives.

Revisit and Revise: Language is constantly changing. This dictionary isn’t static. Encourage students to add new entries, refine definitions, and even note when certain phrases fall out of use.

This activity directly addresses the “why” of idiom instruction. It’s not about memorizing a secret code; it’s about actively engaging with the concept of non-literal language in a relevant, meaningful, and empowering way. It teaches them how to learn these phrases, rather than just what they mean.

Welcome back, fellow SLPs! In our last two posts, we tackled the crucial topic of AI and Clinical Data, as well as AI Truths & Myths. We learned that responsible AI use starts with safeguarding our clients’ privacy and understanding how AI works in practice.

We’ve debunked some common myths and explored the complex questions surrounding AI and data security. Now, let’s pull back the curtain and take a look at the mechanics room. In this post, we’ll demystify how AI tools actually work, providing a clear and simple explanation for SLPs.

Artificial intelligence can sometimes feel like magic, especially when it comes to generating coherent text for notes, therapy ideas, or reports. You type in a prompt, and seemingly out of nowhere, clinically relevant content appears. But behind this impressive capability lies a fascinating, albeit complex, system built on specific AI principles.

As SLPs, understanding the basics of how AI tools function is essential. It can not only alleviate some of the mystery, but also empower us to use these tools more effectively and with realistic expectations. So, let’s peek under the hood and demystify the mechanics of AI.

The Foundation: Large Language Models (LLMs)

To truly understand how AI works, you must first understand its base. At the heart of most AI tools you encounter that generate text are Large Language Models (LLMs). Think of these LLMs as incredibly advanced prediction machines for language.

They’re not thinking, feeling beings; instead, they are sophisticated algorithms. They are often built on a “transformer” architecture, that have learned to predict what word or sequence of words is most likely to come next. This ability to predict is the foundation of how they generate paragraphs, articles, notes, and even initial drafts of reports.

The Training Process: Learning from Text Patterns

The secret to this predictive power lies in the training process. LLMs are trained on truly massive amounts of text data – billions upon billions of words scraped from books, articles, websites, code, and just about any other digital text you can imagine. For models used in healthcare or clinical research, this could also include vast quantities of de-identified research papers, medical journals, and clinical guidelines.

During this training, the AI doesn’t process this raw text directly. Instead, it goes through a crucial step called tokenization.

What is Tokenization?

Tokenization is the process of breaking down the text into smaller, manageable units called tokens. These tokens can be whole words (“the,” “client”), parts of words (“-ing,” “re-“), or even individual characters or punctuation marks. The way text is tokenized can vary between different AI models, but the fundamental idea is to convert human language into a numerical format that the AI can process.

Think of tokenization like the raw data stream in the movie The Matrix. Instead of seeing words and sentences, the AI sees a constant flow of these individual numerical tokens. This numerical representation is the fundamental language that the AI understands and manipulates.

Each unique token in the AI’s vast vocabulary is then assigned a numerical ID. Therefore, AI operates at this granular level of tokens and learns statistical relationships between them. It doesn’t typically “hold” or store entire books, articles, or clinical records as copies. Instead, it retains the learned patterns and probabilities of token sequences.

How is Tokenization Used?

Once the text is tokenized and converted into these numerical representations, the AI analyzes this vast ocean of language, identifying patterns. It learns the statistical probability of certain tokens appearing together. This includes the common structures of sentences and paragraphs, and typical ways humans express different ideas in various contexts (e.g., academic writing, informal speech, or clinical documentation).

For example, the AI learns that after the tokens “Client” followed by “demonstrated,” the token “progress” has a very high probability of appearing next in a clinical note. It learns the statistical relationships between diagnostic terms and common intervention strategies.

It’s all about recognizing these statistical relationships between these numerical tokens. It’s crucial to remember that the AI doesn’t “understand” these relationships in the way a human clinician does. It doesn’t grasp the clinical meaning, the client’s emotional state, or the unique context of a therapy session. It simply recognizes that, based on the numerical sequences it has processed, certain token sequences are more probable than others.

Mechanics of AI Text Generation: Prediction in Action

So, how does this learned knowledge translate into generated text that can assist an SLP? Let’s discuss the AI principles at work. First, you give an AI a prompt – a request, a question, or even just a few keywords (e.g., “Draft a SOAP note for a 5-year-old with articulation goals for /r/”). Then, the AI first tokenizes your input.

After that, it processes its learned patterns and the numerical IDs of these tokens. It then predicts the most likely next token (represented by its numerical ID). This prediction is heavily influenced by the parameters of the LLM. Think of these parameters, often billions or even trillions, as the internal “knobs and dials”, like the Oz Wizard. The AI adjusts these during its training to learn the strength of the statistical relationships between tokens. The more parameters an AI has, generally the more nuanced and sophisticated its predictions can be.

The Output of the AI Machine

The AI takes that newly predicted token. It compares the original prompt and the token it just produced. Then it predicts the next most likely token, again relying on its vast network of parameters. This process continues, token by token, until the AI determines it has reached a stopping point. Finally, these numerical tokens are converted back into human-readable text.

Think of it like a super-powered version of the autocomplete feature you might use when typing. However, it operates at the level of fundamental text units, and is guided by an incredibly complex web of learned relationships stored within its parameters. The “creativity” or “clinical relevance” we sometimes see in AI-generated text isn’t true clinical reasoning in the human sense. Rather, it is the result of the sheer vastness of the data it has been trained on. It has learned the intricate statistical relationships between these tokens. It is all governed by its numerous parameters.

What AI Isn’t: Understanding the Clinical Limitations

Finally, it’s vital to reiterate what AI isn’t, especially from an SLP perspective. Even understanding the mechanics of AI, its human-like responses can often muddy the waters.

• It isn’t conscious. It doesn’t have emotions, personal experiences, or clinical judgment to draw upon.
• It cannot understand or provide the nuances of human communication. Sarcasm, irony, or the subtle emotional context of a client’s utterance are often difficult for AI to understand. They are even harder for AI to create (don’t ask it to generate jokes).
• It is limited in understanding highly nuanced linguistic elements like prosody, intonation, or specific phonetic realizations (e.g., truly hearing and analyzing a distorted /s/ or a lateral lisp). These areas are critical for SLP assessment and intervention, but challenging for current AI to grasp fully and accurately.
• It cannot build genuine therapeutic rapport or provide the empathetic connection essential for effective therapy.
• It cannot make a differential diagnosis or independently formulate a comprehensive, individualized treatment plan based on dynamic clinical interactions.
• AI can’t handle very large chunks of information. This is a significant limitation to remember, especially when working with long documents. More advanced and often paid AI versions, which are hopefully HIPAA-compliant, can process larger numbers of tokens.

Its output, however impressive, is ultimately based on the patterns it has learned from the text it has been trained on. You must remember that it is processed at the level of tokens and statistical probabilities, not genuine clinical understanding. You MUST always double-check output for accuracy, completeness, and bias!

Looking Ahead: Understanding AI as Your SLP’s Toolbox

Understanding the core mechanics of AI—that it’s a sophisticated text prediction machine built on statistical patterns—is the key to unlocking its potential without falling for the hype. This knowledge empowers us to see AI not as a magical entity, but as a powerful tool in our clinical process. It’s an assistant that can handle routine tasks, potentially allowing us to save precious time and redirect our energy to where it matters most: our clients.

I recently found the free internet browser Brave and have been using it the past couple of weeks.  It was simple to move my Google bookmarks into Brave, and I am using it solely for use with the kids. This streamlines my therapy sessions too.  I haven’t had a single YouTube ad. Also, I haven’t noticed other ads popping up on websites.  I will say that, while it does block the ad on a website, there is often still the space where the ad would be.  Often, a reader extension like ReadBee will remove those spaces and focus on the text.

Ways to Use Pinterest Pictures

Now, we all know the drill with Pinterest – it’s the go-to hub for all things DIY, recipes, and yes, even therapy ideas! But let me tell you how I’ve been using it to spice up my therapy sessions.

Picture this: I’ve got boards filled to the brim with stimuli. They cover everything from articulation practice to critical thinking and social skills. We could get into a 10 minute discussion on whether it is photoshopped or real on some of these. Or discuss what makes it interesting, or brainstorm what you would do if you were walking around and saw it in person.

So MANY speech and language goals can be addressed with these open-ended materials. They are so much more fun than reading a card or doing a worksheet. Your kids may not even realize they are “working.”

Seriously, my Picture Prompts boards are like a treasure trove for your therapy sessions. Once you get used to using them, and the types of questioning you can use, you will find yourself using them more and more. For now, I have them separated into just a younger board and an older board. However, I have so many pins, I am going to have to separate them further. 

You guys have to come take a peek! Just make sure you set a timer because you will seriously get sucked into the rabbit hole – LOL!

Multiple Meaning Words

One thing my kids LOVE is when I open up the Visual Puns board. It’s a great way to work on multiple meanings and figurative or non-literal language. Not to mention the sheer fun of figuring out what makes each pun tick. The engagement and comprehension seems to skyrocket when I address language goals this way. And the best part – it’s so effortless it seems like a free day for both me and my students!

Tech Tips

Here’s the kicker – using Pinterest in therapy takes a bit of tweaking. But once you get the hang of it, it is smooth sailing, I promise. First, make sure your view options are set for Default, not Compact. You can choose this by using the little slider button in the top left of your board, as you can see in this screenshot.

I like to zoom in until only one or two images are visible. This makes those images pop and limits what my kiddos are focusing on. Ctrl and + on Windows, Cmd and + on Mac – you get the drift. Most browsers also have their own zoom controls.

Why Pinterest for Your Speech Sessions?

Now, why go through all this trouble instead of just hitting that good ol’ Save button? Well, for starters, it saves me from diving into the murky waters of copyright laws. Ain’t nobody got time for that! Plus, who wants to spend hours copy, pasting, and formatting when Pinterest does the heavy lifting for you?

And as we all know, adding to our boards is as easy as pie. I’m constantly stumbling upon gems while tumbling down the internet rabbit hole. With a click here and a pin there, my collection keeps growing faster than I can say “therapy time.” So much easier than trying to add to a document or slideshow or however else you try to organize saved images.

So, there you have it – Pinterest in speech-language therapy. Who knew? But hey, when it works like magic, who am I to argue?

Now, I know what you’re thinking: “We’ve all used YouTube with kids before.” But bear with me, because there’s more to it than just plopping them in front of the screen while we sip our coffee (although, let’s be honest, sometimes that’s exactly what we need).

Here’s some of the things I’ve found out there in the big wide ‘Tube.

So…while nothing I share here is exactly news-breaking, I hope I have some ideas and links that give you a bit of a boost to your everyday!

Youtube for motivation

First off, there’s the motivator videos, the ones we can offer as rewards or to have kids request. We all know the kids love the great fun stuff out there, and it can be tricky finding what catches each kid’s attention – and it can change rapidly (because nothing is easy!). Depending on what each student likes or needs, it can range from the catchy, energetic songs…

…to repetitive, predictable, calming sensory videos (seriously, I sit back and watch these and am surprised when I look back at the clock at how much time has passed)…

YouTube for concept teaching

Then we have the fun concept teaching videos. These teach concepts in an enjoyable, engaging way that kids don’t realize they are learning. The repetition of each concept is what makes it beautiful, and more often than not, the children will be imitating by the end of the song.  We have core vocabulary songs…

More and All Done Sign Song

…body part songs…

FUN parts of your body song

…Farm animal songs…

Old MacDonald | Real Animals | Jack Hartmann (Love this one, because so many of our kids haven’t actually seen a real pig, let alone heard one)

But what I like best are the story videos, that offer a treasure-trove of language-rich content.  Some of them are musical, like Pete the Cat and Chicka Chicka Boom Boom…

…And others are simply retold with animation. I love the learning English videos, like the ones from English SingSing…

The Three Little Pigs

I find that switching between all of these, and using a mix of planning and using my super SLP thinking skills (because we all know how these best laid plans often go, especially with kids involved), I can hit so many emerging language goals like attending, labelling, requesting and commenting. 

Pro tips for YouTube in therapy

Interactive Viewing

You can pause the video and talk about it, or wait for a request. Or you can watch the whole thing at one time, then use the precise seeking (or link at different times in the video) to go back to different moments to talk about.

Slow it down

Another great thing about many of the English learning videos is that they are told at a slightly slower rate of speech.  But if they aren’t, experiment with using the speed setting.  I find that going to 75% doesn’t usually affect the quality of the speech, but allows the listener more time to process. 

Split your screen 

Divide your screen into two, as to display two different videos side by side, as seen in the first screenshot at the top of this post. This gives. kids a visual choice so they can see what you are offering and point. You can find screen split options by hovering over the square next to the close window ‘x’, as seen in the partial screenshot to the side.  

Theatre Mode

And if you’re worried about distractions, try Theater Mode view to help keep things focused.  It presents the video screen the entire width of the window, so it squeezes out the ads on the side.  

So if I’m able to to have a few moments to reset things before a session, I have the screen split to show two videos side by side in Theater Mode so the child can make a choice.  

If you select full screen, it will fill the entire screen, regardless of whether it is split or not.  So I use that after the child has chosen.  Then I hit escape when we are ready for another choice, to return back to the split screen.

Reduce Inappropriate Ads

Create a separate “school” YouTube account to help reduce the number of inappropriate ads and video suggestions.  

Advertising Hacks

Now, there are some tricks for eliminating ads from YouTube videos, like adding a dash to the url or using a website or browser extension.  Unfortunately, I’ve found that they either don’t work with the Kids videos or they are so unwieldy to use with the short attention spans that it isn’t worth it.  Not to mention YouTube is actively working to block the adblockers.  But you can definitely try them if you’d like!

You don’t want your videos moving on without you.  Go to the settings icon to turn autoplay off.

Organization Tips for Therapy Videos

Now, I know what you’re thinking: how do I keep track of all these fantastic videos? 

Playlists

Yes, YouTube has playlists, and those are great, if you can use them.  Unfortunately, most of these videos are considered YouTube Kids videos, which means you can’t save them to a playlist.

Quick-Access Links – Essential Hack for YouTube in Therapy

Well, fear not! While you can’t save kid videos to your YouTube playlists, you can easily share them, copy the link, and paste it into a handy document for future reference. Don’t forget to set the start time for an even more streamlined list.

I have a document for my Emerging Language Classes that I keep on Outline Mode, and each of the above videos are under their own headings, so they are easy to find while I’m in a session.  It’s a simple trick that keeps everything organized and ready to roll whenever you need it.

Just like our younger clients, older students love a good reward at the end of a challenging session. The beauty? They’re experts at navigating YouTube, so they can easily cue up their favorite vids. But wait, there’s more! I love introducing them to some lesser-known gems that pack a powerful punch:

Flexible thinking and how the mind learns new things:

Backwards bicycle: this was mind-blowing to watch!

How mistakes are okay and help you learn:

OK Go Rube Goldberg Machine (I talk about how you can see the evidence of how many previous tries – 85 to be exact – they had to get it right, but how awesome it was when they got it right.  BTW, the song itself has a great message of how bad things are only temporary.) It’s also useful to see the TEDTalk on how it was engineered. (Caution, the TEDTalk has one minor cuss word and the band is having a drink of champagne at the end.)  If you don’t know what a Rube Goldberg machine is, it is a YouTube rabbit hole.  I warned you.

The Power of a Stutter

A TedTalk on how accepting your insecurities and even embracing them allows us to become better communicators.  How it frees us from worrying about hiding them to think about how we can speak to and help others. (BTW, she has a very noticeable stutter so this is an excellent video for your fluency students.)

How to make sounds

I’m probably not alone when I say that I turn to these when I have a kid I just can’t get to make the sound, especially now that I am wholly teletherapy.  My camera just isn’t as professional as what I can find on YouTube.  The two best sources for sound instruction I’ve found are Sharon Turner, who has a animated video model of the tongue inside the mouth for each sound, and Peachie Speechie, who has great teaching videos for each sound.  But there are many, many options to choose from.

Read-Alouds

I’m using short stories in most of my older groups.  When we first read a story, I want it to be a dramatic, cohesive reading, so I actually don’t want the students to read it.  I want them to listen and be drawn into the story.   But you know, a day of that and my voice gets tired.  Cue YouTube read-alouds!  You can find an audiobook of most popular short stories on YouTube pretty easily.  And bonus – most you can turn on Closed Captioning so students can read-along if you don’t have a text that matches well.  And the students actually love being read to.  If I have a student who needs to read for a goal, then I can stop and have them read a paragraph or so, then return to the read aloud. Background Knowledge

To go along with the read-alouds, a well-made video to give background knowledge is worth so much more than me droning on about what life in a lighthouse is like.  You know what they say, “A picture is worth a thousand words.”  Well, a video must be worth a million, right?!  And they are so much more willing to watch a video than listen to me (and let’s be honest, it gives my voice a break).

Movies

And if at all possible, I like to find at least a clip of a movie or animation to go with the story.  It provides so many opportunities for language-rich discussion, with opinions (Which did you like better, and why?) comparing and contrasting, and inferencing (Why do you think the director made this change?).

Social Skills

There are SOOOO many social skills videos out there.  Some of them are tailored toward our autistic population, some are more for introverted young adults, and some are for the ESL population.  Don’t just stick to the videos that tell how-to do something.  Sometimes the awkward videos offer great teaching moments too.  This one is one of my favorites.  While I do touch on what he does wrong, I mainly use it to talk about other people’s responses, and how they are generally kind about it and make an effort to not make him feel bad even though everyone feels awkward.  People are typically kind, so you might as well go out there and try.

Pro Tips 

Now, I mentioned in Read-Alouds to turn on the Closed Captioning if you don’t have a text that matches the reading.  It might be useful to do so with the other videos as well, to make the speech/text connection and fill in any missing gaps in the auditory information.  

I also suggest you turn the speed to 75%.  This is usually not noticeable to the kids, but it allows them more time to process what is being said so they get a better understanding of the information.

So a few years ago I started hearing raves in continuing education about how many targets you could reach from reading a picture book or short story in speech therapy.  And I thought, “yeah, but you’d only  maybe get one or two practices out of each book for most of those language goals.  Drilling is far superior.”  Plus…it would take a whole session to read a book.  You wouldn’t have time for anything else.

I kept on with my drilling.  And my students made progress on their little goal chart.  Good progress.  And sure, sometimes teachers would say, “but they can’t do it in my room,” And I’d say, “well, that’s because I’m working on a discrete skill, to make sure they have the ability.  Now we have to broaden it to a longer text.”

Literacy Units – Language in Context

But I started hearing more and more about using literature as an SLP.  I started taking these continuing ed classes that were talking about using a picture book as the anchor for a month of therapy. And it was like there was an actual lightbulb in my head.  “You mean we can use the SAME book for a whole MONTH?!!”  Now THAT, that seemed workable.  In fact, that seemed downright awesome.

Because – context. While my drills were great, and they reinforced the skill, they were like practicing a sound in isolation, and would only take a student so far.  And honestly…sooooo boring. So going over a fun picture book for a month, that sounded like just the thing.

Pick the “Just Right” Story

But I was in a middle school setting now.  And sure, there are picture books that are higher language that would fit the bill for middle school. But it was October.  And now your head is probably going straight where mine went.  How could I not introduce my middle schoolers to “The Tell-Tale Heart”? (OK, it was a draw between that and “Ichabod Crane” but they rolled their eyes and said they read that every Halloween).

Well, first, let me tell you: success hinges on finding the “just right” version.  The middle-schoolers who you see for language are probably not going to do well with the original version of the story.  But you don’t want a version too watered down either, or you lose so much of the lovely language-teaching opportunities.  Another key: do not have the students read the first time.  It seems like a good idea, but if you are reading for meaning, you want an advanced reader who has read the story at least once to read it aloud for the group.

Decide the Order of Teaching

And then…do you pre-teach every vocabulary word and turn of phrase, or do you do a read-through first, then go back over it?  Or, maybe watch a video that had most of the text first, that gives meaning to words they don’t know?  I did a little of everything that first time, each group a little different.  I think it was best when I gave a little minilesson first, then we watched the video, which I would pause every so often to allow them to predict or make sure they knew the big picture of what was happening.  Then we went back, closely, over the text.  This may differ depending on the text you read, too.

Language Skills Galore!

I didn’t spend a month on “The Tell-Tale Heart.”  We spent two, maybe three 30 minute sessions.  Partially because I still have a hard time reconciling my old drill mindset.  But I also only have two in my groups (don’t hate me – it’s a rule for teletherapy where I am).  I also was winging it in large part.  If I had been thoroughly prepared, I would have had more and better questions. 

I’m writing my questions down and keeping documents so next year, I will be better prepared.  If you are interested, you can find my completed guides in my TPT store, Mrs. Speech.  They contain questions addressing context clues, figurative language, sentence writing, predictions, inferences, story craft, story arcs, prefixes and suffixes and other language and literacy skills.

Appreciation of Literature

But the engagement of the kids!!  They LOVED it!  So much that I went out and hunted down short stories in a number of genres, based on what they requested, and found so many lovely stories! 

They are requesting stories now – and they remember exactly where we were in the story from one week to the next.  They ask for more from the same author (yes, they are asking for stories).  And most of these stories I am using are at least 50 years old, some, like Poe, are in public domain.

And I love that I am fostering a love for literature.  Even though when I ask, they say they wouldn’t read the longer version, LOL, they still are listening and responding to these stories with enthusiasm and asking for more.  Which is far more than my drill cards ever got!

In the realm of speech-language therapy, engaging clients in meaningful activities that foster communication skills is paramount. However, maintaining motivation and enthusiasm throughout therapy sessions can sometimes be challenging. By infusing elements of play and competition into therapy activities, gamification not only makes learning more enjoyable but also enhances participation and accelerates progress. 

This is where gamification comes in as a powerful tool, a new term for something that most therapists have been using for decades.  Just about every pediatric therapist uses games like Bingo, “Feed the Monster”, and various board games to help motivate students.  But let’s really delve into how gamification can transform speech-language therapy into a dynamic and effective journey towards improved communication skills.

Goal of the Game

Before diving into gamification strategies, it’s crucial to establish the goal of the game in the therapy session.  Are we wanting to use the games to simply keep the student(s) motivated and engaged during the session, as a reward at the end of the session, or to help them take responsibility for their learning by tracking their correct vs their incorrect answers?  

Motivation and Engagement

For many of our kids, the games are simply a way to keep them motivated and engaged. These may be a range of various board games or free online games like four-in-a-row or snakes and ladders. These can be found at various websites such as calculators.org or coolmathgames.com. abcya.com is a favorite for younger games like making a cookie (following directions or using adjectives) or saving chicks (following/using positional words).

There are some modestly priced subscription sites as well, such as pinkcatgames.com, or ultimateslp.com. These are nice because the speech and language stimulus materials are integrated with the games.  You can make your own Jeopardy style game on Bamboozle for just about any subject (check out my growing collection), or use one of the many pre-made ones.  Educaplay also has many styles of games under the free tier that you can make or search for.  Genially is another platform that has many free game and lesson templates and is FERPA compliant.  Even simple push-button buzzers that kids can use when they know the answer makes question and answer activities more “gamey”.

Self-monitoring or effort

However, some students do really well with a bit more of a “push” in which they are given both positive and negative points (or teacher gets points).  Jeopardy-style, tally score, or scoreboards might be best for these students.  These work amazingly for those kids who are working on self-monitoring skills, or who are not giving thoughtful answers.

Additional Benefits

The beauty of gamification lies in its versatility. You can create a custom folder game based on student interests, if you are working in-person, or you can find free games online easily.  Open-ended games such as snakes and ladders can be used with mixed groups and create friendly competition.

Gamification can also promote collaboration and healthy competition among students. Group activities such as word games, role-playing scenarios, or collaborative storytelling encourage peer interaction and communication exchanges. Additionally, friendly competitions, whether in the form of timed challenges or team-based quizzes, inject an element of excitement and camaraderie into therapy sessions, and great opportunities for practice of social skills.

Gamification holds immense potential to make learning enjoyable, engaging, and effective.

Then life threw a curveball, and I resigned from the schools.  I ended up taking a position with a teletherapy company and providing services to various schools.  I was hesitant at first, but after working as a teletherapist for six months, I can say I completely LOVE it!

Comfort

Need I say it? Yoga pants!!!  Business on top, (slumber) party on bottom, am I right?!!  I had a 40 minute, each way, commute before, now I walk down the hall to my office.  I can run to the bathroom or to get a coffee refill or a snack whenever I need.  And have I mentioned no boogers on my stuff?!  I haven’t had a sick day from any child-borne illnesses so far!

Variety

Not all teletherapists have the variety I do, but I have students ranging from mild middle school students to non-verbal kindergarteners.  I enjoy the different caseloads, although it does mean a bit more prep as there is more diversity in goals and materials.  In the morning I get to talk about animal sounds, and in the afternoon I get to teach /r/, and the next day I’m teaching a great short story to work on language skills.

Use of Time

When I worked in the schools, a good part of my time was spent doing duty, attending teacher meetings, filling out paperwork required by the district, completing screenings I was unable to take any action on, performing before or after school requirements such as parent-teacher meetings whether or not I actually had any parents wishing to meet.  Since I was on a contract, I was paid by year, and these times were all under a blanket “duties of the position” statement.

As a teletherapist, I am paid for indirect time as well as direct time.  This differs somewhat from one teletherapy company from another, but from my, admittedly limited, experience, this is typical.  I only attend meetings that directly affect my students.  I feel like I am able to return back to my actual occupation!

Freedom of Scheduling

Along the same lines, I get to draw the lines of my schedule.  I don’t want to work past three?  Then I say I’m not willing to.  I only want to work 30 hours a week?  Then that’s what we shoot for.  I’ve also found that the schools that I work with are more willing to work with me as far as scheduling than I experienced when I worked in a district.  Perhaps I’m just in a sweet spot?  Time will tell.  But I’m loving it right now!

Less Drama

All those powerplays that you couldn’t keep out of in the schools are far less apparent as a teletherapist.  Because we aren’t physically there, we don’t hear the conversations in the halls or the teacher’s lounge. It’s much the same as being at a school part time, where you see your handful of kids and leave.  This, for me, was a fantastic load off my shoulders, because that kind of conflict stresses me out – and there was always someone who was running to me complaining about something.  Now, this can be a drawback as well, especially if you thrive on those connections.  But you can also make connections with your team of teletherapists.

Efficacy of Therapy

Now, do I think I can do therapy as well through a computer as I can in-person?  Yes!  Now, that being said, there are, of course, some caveats.  The method of therapy should be considered for each client just as the time, length and frequency of therapy should be considered.  Often, as my younger students have, a facilitator may be present to aid the client in attention and accessing the technology. 

Other clients may need a different service model that is more of a parent training model.  A setup that includes a separate camera that shows the student’s AAC device so you can see their key presses, or other modifications might be necessary.  With flexibility and creativity, there really is a lot more you can do in teletherapy than you ever thought!

Drawbacks

Ok, yes, I miss the goodies in the teachers’ lounge.  And I REALLY miss the hugs and actually seeing my kiddos.  Sometimes, it is challenging when I have a student that just can’t get a specific speech sound…but I’m not sure being in person would help that either.  Tech issues can be frustrating, when the microphone isn’t catching a nuance of sound or the connection isn’t good.  But there were also times when the hallway outside my room, or the room beside my room, or the restroom beside my room, were so loud that I had trouble then too.  So I might be comparing apples and oranges there.

I’m not necessarily writing this to convert any of ya’ll from in-person therapists to teletherapists, but definitely, should it come to you, don’t be afraid of it.  It could be your next great thing!
